PINK MEANS RETURN TO LONG TRAIL LTS

When you receive a pink form from LTS it means that it needs to be completed and returned to LTS.

New families receive the following forms in their Welcome Aboard packet in late May.  They must be returned to LTS prior to the first day of school.

HEALTH & EMERGENCY CARE FORM  – Lists any health information such as allergies and medications as well as emergency numbers and permission to treat and give over the counter medications.

PHOTOGRAPHY/VIDEO RELEASE – LTS must have on file in order for your child’s photo or a video with your child in it to be used in LTS promotional/advertising materials.

PARENT & STUDENT HANDBOOK AGREEMENT – Please read the Student Parent Handbook online then sign.

PARENTAL PERMISSION AND RELEASE/IDEMNITY FOR STUDENT PARTICIPATION IN FIELD TRIPS AND SERVICE DAYS – LTS must have this form on file for your child to participate in any off-campus LTS event/activity including fall trips.

STUDENT RIDING WITH STUDENT DRIVERS PERMISSION – This is an optional form but needs to be completed in order for your child to leave LTS with a student driver.  Parents can list certain drivers a student is allowed to leave with.

STUDENT DRIVING & PARKING PERMISSION – Grades 10-12 only.  Any student driver wishing to park at LTS must complete.  It also gives permission for your child to transport other students from LTS via their personal vehicle.
