WAIVER AND RELEASE OF CLAIMS
The trip to which this Release shall apply is scheduled to occur March 25-April 4, 2026.

I understand and acknowledge this trip involves air travel, and outdoor physical activities. I acknowledge that there are risks to my child associated with going on this trip.

I hereby authorize a representative of Long Trail School to consent to any emergency medical treatment for my child on this trip, and I further agree to be responsible for any medical, dental or hospital fees associated with such treatment.

I understand that my child is expected to behave responsibly and to follow all LTS rules and policies, and any and all directives of LTS staff or other chaperones on the trip. I understand and agree that if my child does not follow all LTS rules and policies and chaperone directives, my child may be sent home immediately and I will be responsible for any additional costs.

I agree to waive, release, indemnify, hold harmless, and forever discharge Long Trail School, its teachers, administrators, trustees, supervisors, agents, employees, from all claims, demands, liability, right or causes of action of whatsoever kind or nature, brought my myself or any other person or entity, arising from or in any way connected with my or my child’s participation in this trip, including but not limited to negligence, gross negligence, personal injury or loss of any kind, property damage, economic loss, court costs, attorneys’ fees and interest. This release relates to any risks whether foreseen or unforeseen.
