I/We, listed below, am/are the parents and/or lawful guardians of the Long Trail School student indicated on this form. I/We acknowledge and agree to allow him/her/them to ride with Long Trail School student or faculty drivers in their private vehicle. I/We understand and acknowledge that Long Trail School disclaims any and all responsibility for the safety and transportation of the driver or the passengers as Long Trail School is not able to control or to provide for safe transportation when private transportation is used.
